INSTRUCTIONS FOR OBTAINING ENTRY PASSES TO MILITARY RESERVATIONS
1.  Instructions for A-Es requesting long-term entry passes (more than 30
days) for personnel and vehicles to military reservations in connection
with PACNAVFACENGCOM contracts are listed below.  Submit requests to:

a.  ENTRY PASSES. Request for entry passes to Navy Region, Hawaii
(formerly Pearl Harbor Naval Base), Ford Island, and Lualualei must be
submitted on A-E's letterhead and include the A-E's current address and
telephone number, the A-E Contract Number, the title of the contract, the
name and telephone number of the PDE, and all information required below.
Subcontractors must complete and submit their own requests.  For A-Es
working on classified contracts, requests for passes must also include the
DOD Facility Security Clearance data (degree, date of clearance, FSC No.,
and by whom granted) before passes will be processed.  A separate original
request for entry is required for each activity not within the confines of
the Navy Region Hawaii.
All civilians operating or riding in a private motor vehicle on naval
installations MUST wear seat belts.
(1) NAVY REGION HAWAII
(a) PERSONNEL PASSES.  Submit original request utilizing the
format specified in enclosure (1) to this Appendix.  Request must be typed
and signed in blue ink. The ADDRESS column is for Home Address, not P.O.
Box No., not business address.  Please include zip code. Place of birth
must include city and state.  Insure naturalization/alien registration
number is included where applicable.  ALL foreign births must be explained;
i.e., born to U.S. parents, etc.
(b) VEHICLE PASSES.  Submit original utilizing the format
specified in enclosure (2) to this appendix.  Original must be typed and
signed in blue ink.
(c) The Naval Station Pass and I.D. Office is located outside
the Nimitz Gate, Building 3455.  The hours of operation are Monday through
Friday 7:30-11:00 a.m. and 1:00 to 3:30 p.m.; Wednesday closed at ll:00
a.m.
(d) Return expired passes or passes that are no longer required
to the Naval Station Pass and I.D. Office, Box 30, Pearl Harbor, Hawaii
96860-5020.
(e) Passes are not transferable.  When vehicles with passes are
sold, that pass shall be returned to the Navy Region Hawaii Pass and I.D.
Office and not transferred to the new vehicle.
(2) PEARL HARBOR SHIPYARD CONTROLLED INDUSTRIAL AREA (CIA).  This
secure area is within the Navy Region Complex.  To gain access, you must
obtain both personnel and vehicle passes from NAVSTA Pearl Harbor and a
