and size of the vehicle(s) (pickup truck, crane, or forklift), parking
location, and purpose and duration.
(k) Photography is prohibited anywhere in the Pearl Harbor
Naval Shipyard.  This includes spaces outside the CIA.  When operationally
required, submit a written request containing specific justification and
details to the Security Services Administration Branch (Code 1125) via the
Contracting Officer for consideration.  Photographs shall be reviewed by
the Security Officer prior to release.
(3) FLEET AND INDUSTRIAL SUPPLY CENTER (FISC), PEARL HARBOR
(a) All visitors conducting business at FISC Pearl Harbor are
required to report to the Security Officer (FISC 02), 473-3727 (FAX 474-
4636) Building 475, first floor for clearance and issuance of visitor
badges.
(b) Personnel List.  Prior to visit, the PACNAVFACENGCOM
Project Design Engineer (PDE) will forward to FISC 02 a copy of the A-E's
entry pass request for Pearl Harbor Naval Base, listing personnel requiring
entry, in accordance with paragraph 1.a above, and the A-E's letter
indicating dates of anticipated visit.
(4) MARINE CORPS BASE HAWAII (MCBH KANEOHE BAY)
(a) Passes issued by other military installations will not be
honored.  All visitors conducting business at MCBH Kaneohe are required to
obtain a temporary (one-day) pass at the Main Gate by presenting:  1)
Driver's license, 2) automobile registration, 3) safety check, and 4) no-
fault insurance.  (If necessary, visitor's sponsor may call the Main Gate,
telephone 257-2047, giving appropriate information.)  (Other procedures
apply to contractors/subcontractors who are required to contact ROICC MCBH
for instructions.)
(5) NAVMAG LUALUALEI/NRTF
(a) Personnel Passes. A NAVMAG access badge is required for all
personnel that routinely visit or work at the Activity, whether for a
duration of one day or one year and longer.  Prior to visit, provide a memo
request (on company letterhead) for personnel passes and vehicle passes
listing names of visitors, social security nos., purpose and duration of
visit to your respective Project Design Engineer (PDE) or PACNAVFACENGCOM
CMP016.  Requests will be faxed (668-3013) to the Pass and ID Office
(Attention Sharon) between the hours of 6:00 a.m. to 2:00 p.m. weekdays.

(6) MIDWAY ISLAND
(a) Submit completed DD 398 Personal History Statement (see
enclosure (7) for a sample) and travel information; i.e. names of
travelers, SSN, name of company, duration of trip, etc..  All numbered
items must be completed.  If numbered item does not apply, indicate "NONE".
Items 16, 17 and 18 must be answered fully.  This form is available from
PACNAVFACENGCOM Code 01C, Security Office.
(b) Entry authorizations are granted by Navy Region, Hawaii
(Code N10).  An endorsement letter by PACNAVFACENGCOM (CMP016) Security
